CASE MANAGEMENT POSITION

We are searching for exemplary Case Managers who can deliver high-quality, targeted mental health case management and rehabilitative services to support individualized recovery and resilience for adults and/or youths in home/community living. Mental health targeted case management and mental health rehabilitative services are intended to improve or maintain a member’s ability to remain integrated in the member’s community Average caseload is 10 – 12 consumers.

Job Related Duties:

  • Meet with the consumer in the community/home- based setting and assist them in reaching their treatment plan goals.
  • Assist our consumer with gaining and coordinating access to necessary care and services appropriate to their needs.
  • Collaborate with a therapeutic team of health care providers assist the consumer to maintain optimal mental and physical health. Provide mental health rehabilitative to address challenges resulting from serious mental illness and affecting the consumer’s ability to develop and maintain social relationships; obtain occupational or educational achievement; develop and implement independent living skills; or obtain housing.
  • Collaborate with the patient and the healthcare team to develop comprehensive treatment plans that address the individual’s psychiatric needs, goals, and preferences. Ensure that treatment plans are evidence-based, person-centered, and culturally sensitive.
  • Provide psychoeducation to patients and their families regarding their psychiatric conditions, treatment options, medication management, coping strategies, and self-care practices. Empower patients to actively participate in their treatment and make informed decisions.
  • Assess and respond to psychiatric emergencies or crises, providing immediate support and coordinating appropriate interventions. Collaborate with emergency services, crisis hotlines, and psychiatric emergency departments as needed.
 

Minimum Requirements:

  • Must be at least 21 years of age, physically, mentally, and emotionally capable of performing assigned tasks and have the skills necessary to perform assigned tasks.
  • A Bachelor’s degree or higher from an accredited college or university in social work, psychology, nursing or other human service field. Credit hours in graduate level courses that focus on family and individual function and interaction desired.
  • QMHP-CS certification eligible One year of case management experience, including fieldwork in consumers’ homes.

Psychiatrist Specific Job Responsibilities:
